The SF 3112 parts have distinct owners

SF 3112 is the documentation package, not the name of just the applicant statement. The correct components are:

“SF 3112-1” and “SF 3112-2” are not the labels for the physician and supervisor statements. Use A through E to avoid requesting the wrong document.

Start through the current application system

OPM now directs FERS disability applications through ORA. Current employees begin with HR; separated employees use ORA's former-employee disability option. Retirement application information, benefit elections, disability documentation, and supporting agency documents are all part of the process. Follow the current ORA checklist rather than using an older paper packet's mailing route.

Prepare useful evidence

Give your treating provider the position description and explain the actual demands of your work. Obtain current documentation of diagnosis, clinical findings, treatment and response, functional restrictions, and whether the disabling condition is expected to continue for at least one year from filing. The provider supplies the medical opinion; applicants should not invent findings or limitations.

Current, relevant evidence is more useful than a large undifferentiated file. Follow the instructions shown in ORA and any specific OPM request. Do not assume more pages improve an application or discard older evidence needed to establish onset or history without considering the actual request.

Check the other requirements

Request your agency portions early, including after separation. Address the Social Security disability application requirement and retain proof. Confirm your separation date and filing deadline; do not let unanswered requests run out the one-year window. Keep copies of your submitted application and its confirmation.
