Consultation service agreement
Between The Federal Disability Review, LLC (TFDR) and the person booking the consultation. Agreement version: TFDR-CONSULT-2026-10-16.
1. Your consultation
The fee is $75 for one scheduled telephone consultation lasting up to 60 minutes. Emily Hall will call the callback number supplied at booking. The consultation provides education about the federal disability retirement process, discussion of client-prepared materials, and practical preparation guidance.
2. Service boundaries
TFDR is an independent non-attorney consulting business, not a law firm or government agency. This service does not include legal advice, medical opinions, representation before OPM or MSPB, preparing or submitting an application on your behalf, or a guarantee of eligibility, approval, processing time, or benefits.
3. Your responsibilities
You remain responsible for your decisions, accurate information, application preparation, filings, and deadlines. A consultation does not extend any deadline. Obtain advice from a licensed attorney or qualified medical professional when needed. Be available at your scheduled time with a working callback number.
4. Payment and additional services
The $75 fee covers this consultation only. Records organization and the application support program require a separate purchase and agreement. There is no automatic enrollment or recurring charge. Payment is collected through FreshBooks.
5. Cancellation and refund terms
- Cancel at least 24 hours before the appointment: full refund.
- Reschedule at least 24 hours before the appointment: no charge.
- Cancel within 24 hours or miss the appointment: no refund, with exceptions at TFDR’s discretion.
- If TFDR cancels: choose a full refund or rescheduling.
- If you are late: the call ends at the original scheduled time.
6. Privacy
Provide only ordinary contact and scheduling details when booking. Do not include medical records, diagnoses, Social Security numbers, or sensitive case details in booking or payment fields. See the privacy policy.
7. Agreement acceptance
Review these terms before booking. In the Google Calendar booking form, type your full legal name in the electronic-signature field to indicate your intent to sign and agree to this consultation agreement, version TFDR-CONSULT-2026-10-16. Your signature is submitted with your booking details. Retain a copy of these terms for your records. Contact TFDR if you prefer to arrange a paper agreement before booking.
