Service Agreement
Review the scope and limitations before choosing a paid service.
Consultation terms are available in the separate consultation agreement. Other service terms remain under preparation.
1. Parties and scope
I request educational or administrative consulting from The Federal Disability Review, LLC (TFDR). My selected service and price will be stated before checkout. This acknowledgment does not expand the agreed scope.
2. Independent non-attorney services
I understand TFDR is not a law firm or government agency and does not provide legal advice, representation, medical opinions, or guarantees of eligibility or approval.
3. $75 consultation
A $75 consultation provides one scheduled telephone meeting of up to 60 minutes for process education and review of client-prepared materials. Emily calls the callback number provided at booking. TFDR does not complete or submit applications on my behalf.
4. Medical records organization
When purchased, TFDR organizes supplied records into a bookmarked PDF, factual medical chronology, and factual summary for my treating physician to review. The physician independently determines medical findings and completes the physician statement. Standalone pricing is $175 for 1–100 supplied pages, plus $1 for each supplied page over 100, with the same formula above 500 pages. All supplied pages count. No quote is required. Scanning, unreadable-file repair, and major revisions are not included. Secure transfer instructions will be provided separately.
5. $1,800 initial application program
Includes up to 26 weekly consultations of up to one hour over no more than six calendar months from the agreed start date, plus records organization up to 350 pages. It ends at the earliest of initial OPM application submission, six months, or 26 completed meetings. No automatic extensions or unlimited messaging. Reconsiderations, appeals, and legal representation are excluded.
6. Client responsibilities
I am responsible for my information, documents, deadlines, preparing and submitting my application, and seeking independent legal or medical advice when needed.
7. Service-specific terms
For $75 consultations, review the consultation agreement for the approved cancellation and refund terms. Records organization and the application support program require separate finalized agreements before enrollment.
8. Electronic acknowledgment
Consultation agreement acceptance is collected through the required typed-name field in the Google Calendar booking form. This general service page does not collect signatures.
